FINANCIAL ARRANGEMENT OPTIONS:
A.	Total fees paid at time of service by credit card, check or cash.
B.	Total fees paid in advance (by check or cash only ) a 5% billing courtesy is given on total fees of cases $5,000.00 or more.
C.	Arrangement of payments in installments on certain cases with multiple visits.
